Amica Medical Group of Companies authenticates all credit card transactions.  We also cooperate with local and Federal authorities to prosecute every incident of credit card fraud to the fullest extent of the law.

Wiring Instructions
For bank wire instructions, contact our sales team at 1-888-939-0774, or our contact page. Please note that on orders below $1,000.00 US, we must charge a $25.00 wire transfer fee.

Purchase Orders
Government, educational and corporate customers may establish Net 30 Day terms with Amica Medical Group. Please download and complete a credit application. Then fax your completed application, along with your purchase order, to us at 1-888-939-0774.

We require the following minimum order amounts for all first-time purchase orders:

  • $100.00 from schools and government entities
  • $250.00 from corporate customers

Subsequent orders may be of any denomination. Thank you for your understanding.

Orders originating from OR shipping outside the USA and the US Virgin Islands:

Due to a high percentage of credit card fraud related to offshore orders, we can only accept wire transfers for customers residing outside of the USA.  For long-term customers, we may make exceptions.
